Submit Your Immunization Form

COVID-19 Vaccine

As of June 2022, the U.S. Centers for Disease Control and Prevention requires travelers, including international students, to have proof of a COVID-19 vaccine in order to enter the United States. Review our international travel page for updates.

The University of Minnesota also has a COVID-19 vaccination requirement. You will need to provide proof of vaccination or provide a sincerely held religious belief exemption or a medical exemption. You can submit your COVID-19 vaccination information by visiting your MyU Portal and locating the ‘My Info’ tab.

Review the University of Minnesota's Get the Vax website for up-to-date information about the UMN's COVID-19 vaccine requirements.

Additional Immunization Requirement

Minnesota law requires all students to be immunized against measles, mumps, rubella, tetanus, and diphtheria. A COVID-19 vaccination requirement is also currently in place. Review the immunization requirements and complete the online or printable immunization record available on the Boynton Health website.

International students will not be required to complete this immunization requirement until they arrive on campus (but the COVID-19 vaccine is required before travel to the U.S.). To avoid having a registration hold added to your record, you must submit an immunization form by the sixth week of your first semester.

If you have already received your immunizations, you can complete the online or printable immunization record available on the Boynton Health website. You will need to know the month and year when you received each immunization to complete the form.

If you need one or more immunizations, you can choose whether to get your vaccinations in your home country or after you arrive at UMN. There is a health clinic on campus called Boynton Health, where you can get the required immunizations at no cost to you if you have the Student Health Benefit Plan (student health insurance).
