Submit Your Immunization Form

Minnesota law requires all students to be immunized against measles, mumps, rubella, tetanus, and diphtheria.

Review the immunization requirements and complete the online or printable immunization record available on the Boynton Health website.

International students will not be required to complete this immunization requirement until they arrive on campus. To avoid having a registration hold added to your record, you must submit an immunization form by the sixth week of your first semester.

If you have already received your immunizations, you can complete the online form from Boynton Health. To complete the form, you will need to know the month and year when you received each immunization.

If you need one or more immunizations, you can choose whether to get your vaccinations in your home country or after you arrive at UMN. There is a health clinic on campus called Boynton Health, where you can get the required immunizations at no cost to you if you have the Student Health Benefit Plan (student health insurance). It is very easy to get vaccinated at Boynton Health. You can come to the drop-in clinic or make an appointment. It is recommended you try to get this done as soon as you arrive, but you do have until the sixth week of class to complete the requirement.

COVID vaccination is not required by the University of Minnesota or for entering the United States, but it is highly recommended.
