Submit Your Immunization Form

Minnesota law requires all students to be immunized against measles, mumps, rubella, tetanus, and diphtheria.

Review the immunization requirements and complete the online or printable immunization record available on the Boynton Health website.

International students will not be required to complete this immunization requirement until they arrive on campus. To avoid having a registration hold added to your record, you must submit an immunization form by the sixth week of your first semester.

If you have already received your immunizations, you can complete the online or printable immunization record available on the Boynton Health website. You will need to know the month and year when you received each immunization to complete the form.

If you need one or more immunizations, you can choose whether to get your vaccinations in your home country or after you arrive at UMN. There is a health clinic on campus called Boynton Health, where you can get the required immunizations at no cost to you if you have the Student Health Benefit Plan (student health insurance).

As of July 1, 2023, COVID-19 vaccination at the University of Minnesota will no longer be required, although we strongly encourage COVID vaccinations and boosters. As of May 12, 2023, noncitizen nonimmigrant air passengers no longer need to show proof of being fully vaccinated with an accepted COVID-19 vaccine to board a flight to the United States.
